Shares of PACS Group surged 7.8% to $50.30 in pre-market trading after the post-acute healthcare operator delivered a third-quarter earnings beat and raised its full-year guidance — a one-two punch that forced investors to reconsider whether the stock has been undervalued. PACS Group Delivers a Blowout Quarter and Lifts Its Outlook Again — but Can Earnings Growth Outrun Federal Investigators?
Shares of PACS Group jumped 7.8% to $50.30 after the post-acute healthcare operator posted third-quarter results that handily topped Wall Street expectations and raised its full-year guidance for the third consecutive quarter this year. The question now: whether the company's operational momentum can keep overshadowing unresolved federal probes.
Earnings Grew Nearly Three Times Faster Than Revenue
Revenue rose 11.5% year over year to $1.50 billion, adjusted EPS climbed 37% to $0.63, and Adjusted EBITDA — a measure of operating profit before certain non-cash charges — surged 33.6% to $175.7 million.
Revenue beat analyst estimates of $1.46 billion, while adjusted EPS of $0.63 exceeded the $0.57 consensus. That lopsided profit-to-revenue ratio signals the company is squeezing more dollars from each bed it fills, not just adding beds.
Fuller Facilities Are Driving the Beat
On a same-store basis across its 284 skilled nursing facilities, revenue grew 6.4%, while occupancy improved to 91.3% from 89.3% a year earlier, with a richer mix of higher-paying skilled patients.
Mature facilities hit 93.1% occupancy, far above the 76.2% at newer locations. That gap is critical: as recently acquired properties ramp up, they offer a long runway of margin improvement without major new spending.
Management Keeps Raising the Bar
PACS lifted full-year 2026 revenue guidance to $5.95–$6.05 billion and Adjusted EBITDA to $685–$695 million, up from prior ranges of $5.75–$5.85 billion and $640–$660 million.
The revised EBITDA range includes roughly $5 million from retroactive Ohio quality-incentive payments , meaning the organic raise is still substantial. PACS also completed acquisitions of 11 Texas and 32 Florida facilities totaling 4,049 beds , fueling forward growth.
Federal Probes Remain the Wild Card
Ongoing DOJ and SEC investigations and an active securities fraud lawsuit remain unresolved risks.
The company's internal investigation has concluded, but the government inquiry — triggered by a November 2024 short-seller report alleging Medicare billing irregularities — remains open.
The company has also acknowledged material weaknesses in internal financial controls. No financial liability has been estimated, leaving a binary risk that no earnings beat can price.
The earnings call at 11:30 a.m. ET today will be closely watched for any updates on the investigations and acquisition integration.
